To submit your application…

Please review the Facilities Request Guidelines before filling out your application. Fill out a Facility Request Form. The form should be filled out completely by the Event Leader and/or delivered to the office at least two weeks prior to the event, with the appropriate deacon/deaconess or Ministry Team leader initialing their consent if applicable.

After We’ve Received Your Form…

The office will verify and approve the calendar date. The office will submit the form to the Properties Managers for their review. They will initial their acknowledgement. It will then be given to the Elder Board for their approval/initial. After it has been initialed by the Elder Board, the office will schedule it. You will be notified and receive an approved copy of your request.
